  • Release and Liability Form
  • Information Form (this is a 2-page document)
  • Full payment by check or Venmo
  • Directory Form
  • Photo and Video Release Form
    All forms (and payment if applicable) may be sent to 184 Pratt Street, Fairfield, CT 06824.
    Your space is considered CONFIRMED only after you receive notification back from Deep Play
    for Kids indicating the same. Space is limited. Once the class is filled, we will have a waitlist.

DPK is proud to offer tiered tuition pricing as follows:

Check made payable to Deep Play for Kids, LLC. Mail payment to: Deep Play for Kids, LLC 184 Pratt Street Fairfield, CT 06824

All payments are nonrefundable unless the program is canceled by Deep Play for Kids, LLC (“DPK LLC”). DPK LLC will not be responsible for refunding airline tickets, train tickets or hotel reservations under any circumstances.

If the registrant cancels 15 days or more before the start of the program date and DPK LLC is notified in writing, the sum of $200 will be deducted by DPK LLC as an administrative expense and the balance after that shall be refunded. By submitting this form and making payment, I, the registrant, agree to these terms and conditions.
