and valuable consideration, do hereby release and waive and discharge Deep Play for Kids, LLC
and employees of Deep Play for Kids, LLC, individually and collectively, of and from all claims and
demands whatsoever which I now have or may have against all of the aforementioned, in regard
to any and all matters relating to photos and/or videos taken of me/my image during the Deep
Play for Kids program that I attended, including, but not limited to, the use of said photos for
training purposes, website, and future use of said photos. I am aware that this program is being
broadcast privately on zoom for DPK attendees.
